To me it looks more like a tumour than an abscess or ingrown quill. I would have it removed and sent for testing. Even if it turns out to be an abscess it still needs to be removed. There...

Some abscess have to be removed if they keep coming back. Most can just be drained and treated with antibiotics but some have to be surgically removed to prevent them from reoccurring.
