ADVICE NEEDED URGENTLY! Hedgehog found, seems unwell, not sure?!

My dad was coming home from work and spotted this hedgehog hobbling down the road, we've had resident wild hedgehogs living in the garden before, so he must have thought if it was limping and looked unwell, maybe we could help it out (I know this is ethically wrong and taking it out of its environment sucks, but he meant well).
He put it in an open box in the car and brought it home, and all the way home it was making croaking (best description for it?) noises, and now it really looks unwell. I took a short video to try and get some help and advice on what we can do for it?!

He is in really really bad shape and needs to get to a rehab center asap. They have the knowledge and medicine that he will need to recover.

It looks like he's dying. Bringing him to a rescue centre or experienced vet is the only thing you can do, if it's not too late already.
